A Correlative Analysis of Social, Economical, Educational and Political Changes and Adolescent Mental disorders in Korea


Abstract
To evaluate a correlative analysis between annual rates of hospitalization and social, economical, educational and political changes, data were collected from 2,140 hospitalized psychiatric adolescents including 1,151 boys and 959 girls who
admitted to
Department of Psychiatry in pildong Hospital and Yongan Hospital, Chung Ang University Medical Center from January 1968 to December 1994.
Among these cases, 725 cases including 430 boys and 295 girls who admitted from January 1987 to December 1994 were selected in the second analysis in order to evaluate clinical psychiatric changes of the adolescent mental disorders according to
the
years of admission. And summary of the results were as follows.
1) The proportion of boys were more in the adolescent age group than girls, Thile female patients were more in the other age group than male cases. In the annual trends of the psychiatric adolescent in-patients, the relative proportion of
admission was
more increased in the female cases, and was more increased in the younger adolescents in recent years.
2) The rate of adolescent psychiatric in-patients among total psychiatric in-patients who admitted from 1968 to 1994 were 15.65%, and these rates were higher in the males as 17.64% compared with females as 13.41% The rate of adolescent
psychiatric
cases among total psychiatric cases and the rate of examiners among adolescent psychiatric cases were more decreased in recent years.
3) Annual rates of admission among hospitalized psychiatric adolescents were closely corelated with the contemporaly social, economical, educational and political changes.
These rates were more decreased in the social atmosphere such as the educational and political refreshing atmosphere, the educational alleviation period, the liberalization free atmosphere, the periods of peaceful climate due to calm down
international
fighting, and the political-economical wellbeing period. While these were more increased in the social atmosphere such as the political turmoil status, the birth of the new republics, the reorganization of educational policies, economical crisis,
the
opening years of olympic games, and the international fighting phases.
These findings suggested that the adolescent mental disorders were higher in the social atmosphere such as the economical, educational and political turmoil status, while these rates were lower in the social, economical, educational and political
wellbeing period.
4) Personality characteristics such as the anxious including avoidant trait or passive aggressive trait, the neurasthenic, the introverted, the expressive, and the love-seeked trends were more increased in recent years, while personality ones
such
as
the hysterical, the hypochodriacal, the melancholic, the hypomanic or the schizophrenic trends were more decreased in recent years.
5) The anxiety and obsessive compulsive compulsive symptoms were more incresed, while the somatzation symptoms, depressed sumptoms, and nervousness were more decreased in recent years. In the subtypes of adolescoent mental disorders. Anxious
manifestations were more increased, while the confused types and the slump types were more decreased in recent years, In the prescribed medication, the anti-anxiety drugs such as diazepam were more increased, while the antidepressants such as
imipramine
or the neuroleptics such as thioridazine were more decreased in recent years.
6) The rates of cases initiated or exacervated due to their heterosexual conflicts or their introverted personality were more increased, while their under-achievement of school performance or domestic problems were mere decreased in recent
years.
In the hospital records, the rate of maladjustment, of suffering from serious psychosocail stressors, of admitted to open ward of discharged from open ward, of hospitalization by forced admission, of readmission, of longger hospitalization, and
of
admitted of August were more increased in recent years.
7) I heir parental resistances toward therapeutic miliew such as the messages of 'I can't talk about my child without talking about myself, and administrative approaches such as physical restraint to manage adolescent's resistances, and the
extinction
tehnique or attack weak point to manage their parentalresistances were more increased, while the adolescent's resistances toward therapeutic miliew, and the orthodox approaches such as supportive psychotherapy, or reassurance to manage
adolescent's
or
parental resistances were more decreased in recent years.
